If you would like to schedule an initial consultation, please call and leave me a voice mail with your full name, phone number, how you were referred, the name of your insurance company and the best time to reach you.  I am often in session all day but will do my utmost to return your call the same day or by the next business day.  If you will provide me with your insurance information I can ensure you have prior authorization and what the amount of your copay may be.
